Parent/Guardian Transportation Authorization
This document grants permission for my child to be transported as part of the Youth Leadership Program.
Transportation Authorization
I give permission for my child to:
• Ride in program-owned or program-approved vehicles (including vans)
• Be transported by authorized youth leaders, staff members, or approved volunteers
• Be picked up and dropped off for program-related events, meetings, and activities

Assumption of Risk
I understand that transportation involves inherent risks, including but not limited to:
• Accidents
• Injury
• Illness
• Property damage

I acknowledge and accept these risks on behalf of myself and my child.

Release of Liability
To the fullest extent permitted by law, I hereby release, waive, discharge, and hold harmless the Youth Leadership Program, its organization, staff, volunteers, leaders, drivers, and affiliates from any and all claims, demands, actions, or causes of action arising out of or related to:
• Transportation to and from program activities
• Participation in Youth Leadership Program events

This includes claims related to injury, illness, accident, or death, except in cases of gross negligence or willful misconduct as defined by law.

Medical Emergency Authorization
In the event of an emergency, I authorize program leadership to obtain emergency medical treatment for my child if I cannot be reached immediately.

Acknowledgment
By signing below, I confirm that:
• I have read and fully understand this document
• I voluntarily give consent for transportation
• I agree not to pursue legal action against the program or its representatives for transportation-related incidents
