Participation Waiver

In consideration of the Youth Corn Maze Event at Beachmont Christian Camp, located at 6433 Mt. Vista Road, Kingsville, MD 21087, on Saturday, November 8, 2025. I understand that Extreme Family Outreach, the staff & volunteers of EFO hold no responsibility for injuries which I may sustain as a result of my participation in this event. I expressly acknowledge on behalf of myself and my heirs that I assume the risk for all injuries and illnesses which may result from my participation during this event. I hereby release and discharge Extreme Family Outreach, its agents, assigns, and/or employees from any and all claims for injury, illness, death, loss, or damage which I may suffer as a result of my participation during this event and transportation to and from Beachmont Christian Camp. I understand that Extreme Family Outreach is not responsible for personal property lost or stolen while members and/or program participants are participating at this event.

I understand that while my child participates in this program, their photograph or video image may be taken. I give my permission for Extreme Family Outreach to use, without limitation or obligation, photographs, video footage, or recordings which may include my child 's image for the purposes of promoting the Extreme Family Outreach program.
